What to Expect
Here’s what you can expect when you choose in-home hospice care from The Denver Hospice:
- A hospice team member will visit your home, assess your needs, advise you on any medical equipment needed and help you make arrangements to obtain it. The Denver Hospice will provide all medications, supplies and other services related to your hospice diagnosis.
- The Denver Hospice care team will create an individualized care plan for you. This helps everyone involved understand the amount of caregiving you require and determine the best way to provide it for you.
- The care plan will designate the team that visits your home. Depending upon your needs, your care team may include a nurse, physician, social worker, counselor, certified nursing assistants, spiritual care provider, therapists and volunteers.
- Hospice team members will visit you regularly in accordance with your care plan and are always accessible by phone if questions arise.
Should your needs or desires for care change, we may consider other arrangements—skilled care facilities, hospitals or our own Inpatient Care Center at Lowry.
