Our hospice services are available to individuals residing at home, in skilled care facilities and assisted living communities or group homes, as well as to those receiving hospital care.
- Skilled care and assisted living facility residents receive the same level of care as individuals living in private residences. Our hospice team works with facility and community staff to create a personalized plan of care. Visits from hospice team members—physician, nurses, certified nursing assistants, therapists and pastoral care counselors—are then scheduled in keeping with that plan.
- Hospital patients who are terminally ill receive care from our hospice team as well as from the hospital staff. Our shared goal is to ensure each individual’s comfort. A member of our hospice team visits each hospital patient daily. When the patient’s condition changes, our team works with the hospital staff to create a plan of care that begins with discharge from the hospital to in an appropriate setting which could be a skilled care facility, assisted living community, group home or private residence.
